Tell me about a launch where the customer experience almost got overlooked because internal teams were focused on their own deliverables. What did you do?

Technical
How do you build a launch plan so that risks and dependencies are visible before they become fires, not after?

What tools or systems have you used to track launch deliverables and milestones across teams, and what did you like or dislike about them?
hands-on familiarity with project tracking tools and whether they think critically about process tooling, not just use it
How do you decide whether a deliverable is actually 'done' and ready to ship, especially for work you can't directly verify yourself?
