Joining AMG means being part of a collaborative, patient-focused environment where innovation and quality care thrive.     We’re Looking For:   • Registered Nurse license issued by the state in which the team member practices   • Bachelor's Degree in Nursing.   • Typically requires 5 years of experience in nursing that includes performing nursing assessments, care plan development, and educating patients.     • Seeking strong neurology background • Must be able to work independently What You’ll Do:   Schedule: Monday-Friday 8:30am-5:00pm No weekends, no holidays • Provides continuity of care by ensuring smooth transitions between care settings .  Develops a relationship with patient and their multidisciplinary team to facilitate and/or navigate through subsequent treatment and follow-up to reflect continuity of care.   • Completes a nursing assessment to identify the patient's individualized needs and collaborates with multidisciplinary team and other departments to develop an interdisciplinary plan of care.   • Supports the patient and family by providing education, addressing psychosocial needs, advocating, providing continuity of care and identifying barriers to care .  Navigates patients and family members throughout the diagnosis, treatment and follow-up of the patient throughout the continuum of care.   • Provides expert clinical advice to the multidisciplinary team in designing collaborative teaching plans/programs and in planning/providing patient/family education and discharge planning including wellness and health maintenance within designated specialty.   • Coordinates timely , seamless, evidenced based care across the care continuum resulting in best patient outcomes.     About This Location:   Advocate Health’s Neurology Clinic in Palos Heights, IL is a highly collaborative, fast-paced outpatient practice dedicated to delivering exceptional, patient-centered neurological care. We value your insights and experiences because they help us grow stronger together.     Ph ysical Requirements and Working Conditions   • Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.    • Must be able to lift and support the weight of 35 pounds in handling patients, medical equipment, and supplies.    • Must be able to: push/pull with 30 lbs. of force and perform a sliding transfer of 150 lbs. with a second person present.    • Must have functional speech, vision, hearing, and touch with ability to use fine hand manipulation skills for IV insertion and other procedures/functions.    • Maneuver foot pedals on carts or machines.    • Perform physical safety interventions such as patient restraint and verbal de-escalation, if needed.    • May be exposed to chemical and hazardous waste as well as blood and body fluids and communicable disease.
