Home Health Registered Nurse Float Pool - Southern WI

Department: 34000 Milwaukee South - Administration: Nursing Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Full Time Home Health Float Pool position based in WI supporting the southern patient service markets. Areas of work based on assignment. Typical schedule is Monday through Friday 8:00am to 4:30pm with some variability based on departmental needs. Every 4th weekend rotation required. Pay Range: $35.50 - $53.25 Major Responsibilities   • Complete scheduled patient visits and collaborate with the interdisciplinary team.  • Conduct comprehensive patient assessments and update plans of care as needed.  • Communicate treatment changes with physicians and the primary registered nurse.  • Monitor patient response to care and revise interventions accordingly.  • Coordinate scheduling and case conferences with primary nurse when appropriate.  • Assists with the discharge planning process according to agency standards and utilizes interagency/system and community resources to ensure continuity of care after discharge.  • Communicates problems to physicians and primary Registered Nurse, and clearly states plans, actions and goals so that changes are reflected as they occur.  • Maintain accurate documentation in compliance with all regulations.  • Follow agency policies and safely perform required patient care activities.  • Provide age-appropriate care based on patient needs and developmental stage.  Minimum Job Requirements  Education • ​ Associate’s degree in nursing. Certification / Registration / License  • Active RN license in state of practice.   • Basic Life Support (BLS) – American Heart Association  • Valid driver's license and reliable transportation.   Experience • 3-4 years clinical experience.   • 1-2 years in home health. Knowledge / Skills / Abilities • ​​Ability to work independently and make sound clinical decisions  • Excellent communication (written and verbal) and interpersonal skills.  • Ability to develop rapport and maintain positive, professional relationships with a variety of patients, staff and physicians.  • Cultural competence and respect for patient diversity.   • Good organization, prioritization and problem-solving skills.  Physical Requirements and Working Conditions • Ability to multi-task with frequent interruptions.  • Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
