whether the candidate understands the shift from a controlled hospital setting to working solo in patients' homes

Tell me about a time you had to document care carefully to support reimbursement or meet a regulatory requirement. What did that process look like?

real experience with documentation standards that affect agency reimbursement, not just general charting habits

practical use of patient education and health literacy skills mentioned in the posting, and adaptability with different learners

Tell me about a time you disagreed with a care decision or noticed something that seemed off, and how you brought that to the RN.

whether the candidate respects the LPN-to-RN supervisory structure while still speaking up when something matters

Tell me about a time you worked with a difficult family dynamic in a patient's home. How did you keep the visit focused on the patient's needs?

interpersonal skill and composure in an unpredictable home environment, distinct from a clinical setting

Technical

How do you use SBAR when handing off a patient to the RN or another team member?

familiarity with a specific communication tool the job description names, and whether they can apply it concretely rather than just naming it

hands-on competency with a specific required skill listed in the posting

What's your comfort level with scheduling and documentation software, and have you used systems like this before?

basic technical readiness for the agency's computer-based scheduling and charting tools

This role involves driving in all kinds of weather and lifting or transferring patients using things like a Hoyer lift or transfer board. Tell me about your experience with patient transfers and physically demanding parts of the job.

whether the candidate has realistic, firsthand experience with the physical demands and safe patient handling required

Situational

Walk me through what you would do if you arrived at a patient's home and noticed a change in their condition that wasn't part of the established plan of care.

whether the candidate knows to consult the RN rather than acting outside their scope, and can articulate the escalation process

Suppose you show up for a scheduled visit and the patient's family is anxious and pushing you to do something outside the RN's care plan. How do you handle that conversation?

ability to hold professional boundaries and use communication skills to manage pressure from families without overstepping scope

How do you plan your day when you have multiple patients spread across a territory like Grayslake, Libertyville, Mundelein, Waukegan, and North Chicago?

time management and organizational skill for a role built around independent travel and scheduling

If you're running behind on a day with back-to-back home visits and a patient needs more time than scheduled, how do you decide what to do?

judgment under time pressure and prioritization skills specific to a full home-visit caseload
