What they are checking: Whether they can organize visits, drive time and documentation without letting any part slide.

Tell me about a time you noticed a patient wasn't progressing toward their goals. What did you change?

How do you keep up with documentation when most of your day is in the field?

What would you do

A member keeps missing doctor appointments because they don't have transportation. How would you resolve that barrier?

What they are checking: Whether they solve practical barriers to care using Medicaid benefits and community resources.

How would you build a plan of care with a member who doesn't think they need help?

What they are checking: Whether they can engage a reluctant member through motivational conversation rather than pushing a plan on them.

During a visit at a long-term care facility, you see something that concerns you about the member's care. What do you do?

What they are checking: Whether they advocate for the member and escalate quality concerns through proper channels.

How do you stay safe when visiting members alone in their homes?

What they are checking: Whether they think about personal safety and know when to reschedule or bring support.

About the work

Walk me through how you assess a member in their home. What are you looking at beyond their diagnoses?

What they are checking: Whether they assess physical, environmental and psychosocial needs, not just the medical chart.
