Likely interview questions for this role

Written from this job description, not a generic list. Each one notes what the interviewer is really checking.

About your past

Tell me about a barrier that kept a patient from getting care and how you resolved it.

What they are checking: Practical problem solving with transportation, pharmacy, housing or provider access, the core of this job.

Tell me about a time you had to make a decision in an ambiguous situation without much direction.

What they are checking: Independent judgment in the field, with good sense about when to seek guidance.

How do you coordinate with physicians, specialists and family members who all have different views on a member's care?

What they are checking: Communication across internal and external stakeholders while keeping the member at the center.

How do you manage a caseload with heavy local travel and still keep documentation timely?

What they are checking: Time management for a field role where late documentation creates compliance risk.

What would you do

During a visit at a supportive living facility, you see the member's medications are not being given as ordered. What do you do?

What they are checking: Clinical judgment, patient safety escalation and working with facility staff respectfully.

A member keeps missing appointments and won't return your calls. How do you approach that?

What they are checking: Persistence and curiosity about the reason behind disengagement, rather than closing the case.

About the work

Walk me through your first home visit with a new Medicaid member. What are you assessing beyond their diagnoses?

What they are checking: Whether they assess physical, environmental and psychosocial factors together, as the posting describes.

How do you write a care plan with goals the member actually agrees with?

What they are checking: Member engagement and shared goal setting, since plans built without the member rarely work.

How do you monitor whether a member is actually progressing toward their outcomes between visits?

What they are checking: Whether they use reassessment and data to adjust plans, not just document visits.

What experience do you have with Medicaid or dual-eligible populations, and what makes care management for them different?

What they are checking: Familiarity with social determinants, benefits and long-term care issues common in this population.
